Investigations indicate that these events have occurred due to lack of supervision, poor safety awareness, inadequate maintenance or failure to comply with pilot ladder procedures of the company’s Safety Management System.

Accidents resulting from incorrectly rigged pilot ladders or, from pilot ladders that are in a poor condition, can have serious consequences, such as severe injuries or the loss of life.
