Vessel’s crew have been observed working in an unsafe manner without using the appropriate PPE when working at heights, working overside when rigging or stowing accommodation and pilot ladders. These unsafe practices have been observed when the vessels are at anchor, while the vessels are navigating in the harbour and when alongside at the berths.
